Yep. She knows that something is there.

The guy was upset when he thought he kissed her but it wasn't her, he tells her there's something going on between them and when she doesn't admit it he says that she's lying to herself, the guy tries to kiss her to prove his point. The guy takes an arrow in the back for her. When he learns that she wants to give herslef up to Klaus, which would mean she's dead, he goes and takes her away from that place. She notices how he's trying to be the "good guy, the better person" and now he tells her "I will always choose you" in a way that expresses everything. In a way that says "I'll always choose you, even over myself".

If at this point she doesn't even suspect it... besides, Damon told her pretty clearly (2x01) that he couldn't forget the kiss and that he feels something for her. What else does she need?

She's blind. Sometimes I feel like Damon is talking to a wall! So far all she has in mind is to stay loyal to the bynny eater. She's in constant denial..
