She's going to make a heartbreaking decision, yes, but who said it's Damon who's going to be heartbroken? Getting too excited for an episode is not good, like 4x04. But expecting the worst long before the episode's air date is not good either. Just wait it out. Besides the heartbreaking confession was supposed to be in 4x07.

Is there any point in speculating about something that probably won't happen? Elena was supposed to make a 'painful' confession to Stefan in 4x06 but it wasn't THAT painful, was it? The synopsis can mislead fans into thinking stuff that is fairly inaccurate when you watch the episode. Like, in one episode last season (I can't remember which one), it said that Damon goes to Bonnie for help about something, and Bamon fans got all excited thinking it was their episode. Totally false. They hardly had any scenes. It may not be what your thinking, is what I'm saying. But even if Damon has to make some kind of decision, it's unlikely he'll give up on Elena, when this season is about them getting closer, and Damon said in 2x18: "I will always choose YOU."

No one said that the heartbreaking decision is about Elena.. I know that it's the most probably option, but nobody said it so.. I'm not even sure about what the heartbreaking decision will be, but he loves Elena so much to giving up on her... I really have no idea

I think the heartbreaking decison will be Damon choosing elena and turnin his back on Stefan because in episode 4x09 Damon and elena will go to the Gilbert boat house with Jeremy.. So they are togther:)
